So, the image below says a fully fused 4* Bajoran Dukat is the top threshold reward. Yet, I see that I can only get three threshold rewards that have Dukat (3000 level, 18000 level, and 120000 level)

Where is the the 4th. Actually, it’s been this was for some time, just now asking the question. It seems that if DB says the Top Threahold is a ff 4*, it should be possI left to get all 4 (Dukat In this case)

So, am I missing something?

Nope — it's just the way DB has always done it. Oddly, in Dabo, when displaying the jackpot prize of the Legendary crew, they only have one of the five stars highlighted. Nobody ever claimed they were consistent.
