Can someone tell me on what occasions a character’s base level for a skill is used, and when a roll is used?
I’ve been looking at some databases, and they list a crew’s base, their min roll, their max roll, and their average roll.
I’m just confused as to what areas of the game these apply to.

Proficiency: Gauntlet
Combined: Away Missions, Voyage
Shuttles: Any shuttle mission, whether daily or event will only use the crews base score. No proficiencies added.
Gauntlet: This will use only the proficiency scores. No bases. So you will only need 1 star on a crew to make them fully effective in gauntlet (adding stars/citing does not increase the proficiency score)
Voyages: A bit more technical, but a voyage score will be each stats Base + Average Roll (so Min+Max divided by 2). Then all those stats added together to give the final score.
Do the voyages stick with the average, or does it perform a roll at each skill check?
That is a good question. I've read here different opinions. I have experienced many times when a particular skill will fail on a voyage one or two or three checks and then pass the next. This makes me believe that proficiency does play a role, though I am not sure how much of a role, nor how significant.
