Yeah, I've had it happen once, if I remember right I got REALLY lucky with that voyage and scored 1 4* for the 4hr, 1 for the 6hr and picked up another somewhere between the 2. Could do with a few more voyages like that, lots of 4* crew I want to upgrade.
Of course if I had them upgraded I'd be more likely to make it to 6hrs and have better odds of getting more 4* crews. Just have to keep using honor to upgrade them. ¯\_(ツ)_/¯
