I think what I'm going to do is make a log of everyone I have active and in cryo, and take screenshots of people list-wise - so like how we can see 6 crew at a time in the roster, just take a photo of the 6 at a time.
I shouldn't have to, but this occurrence, even though it worked out in the end, was a very rude awakening.
This also makes me think each player should have their immortalized date on each of their crew. Sure, we have "first to max", but I may need to know my own maxxing date.

I set Favorite on every character greater than than 1/4 who isn't FFFE or who I just don't want to loose. This prevents accidental deletion. Once they are FFFE, I remove it, since they worst that could happen is they go into cryo.

This is a known bug. A couple of months prior to Cryo Vault I noticed a handful of 2 and 3 stars missing out of freeze. I contacted support and they promptly reinstalled all in my toons. One happened to be a 4 star kim too. I looked pretty hard, wanting to make sure I wasn't mistaken or asking for something I hadnt maxed.
Once the Cryo Vault came out, I found the smaller set of 2 and 3 stars missing - easy to sort. They also put those back asap. Now I dont know if the have records, but on the flip side it is obvious I have ALL 2 and 3 stars immortalized. I am slowly doing the same with 4.
I have wondered if I lost more high levels, and I have been double checking any freeze and unfreeze options now that we can search both sides. So far no new anomalies though. The bug may not be associated with freezing though, cause I doubt I would have froze that kim.

Happened to me when the vault first came out. Lost two 2* crew and a 3*. Took me almost 20 emails and 6 weeks to get them back. Good luck.
Yeah, I had my immortalized Ensign Chekov vanish from my vault never to return once. I didn't file a ticket because he's just a 1* and I got one of him from a basic pack and re-immortalized him in no time.

I've accidently deleted someone on a couple of occasions (most recently last week) and CS has always been able to restore them. It's quicker if you can tell them what level and equipment they had, as that info apparently isn't tracked.
I blame it on the way the crew re-sorts when dismissing. It's easy to mis-click when they are jumping around. I usually go to the re-enlist page to make sure everyone there belongs there.
I have posted this elsewhere in the forums as well. It has become a valuable daily tool I use when playing STT to keep track. I also had several crew go missing last month, and DB didn't even bat an eye about getting them back to me.
Simple spreadsheet that I update almost weekly. It has all next weeks new Event crew as well already on it.
So back before the collections, I started keeping track of all the 1* and 2* crew I had or needed to get. Immortalizing easy crew was a great way to get dilithium (via achievement).
Once I completed that I started recording all my 3* and 4* non-FF crew sitting in my inventory. Being able to get 4stars from voyages and merit pulls has allowed me to immortalize at least a dozen crew I would have just otherwise airlocked or held onto until an event.
Now that Collections are a thing I keep track of 4* and 5* crew like a hawk. I categorize them collection/non-collection then fusion level. For example my 4* list (partial):
3 dr. david marcus - jury rigger, innovator
2 age of sail laforge - innovator
commander kelby - jury rigger
changling bashir - doctor
cardassian kira - cardassian
If I am ever hurting for space, I know who is in danger of airlocking . None of these guys listed are favorited, but keeping a list allows me to know if one is missing. Now, just this morning I got a 4th copy of David Marcus, I threw a heart on him so I see him at the top of my roster and it informs me to FE him so I can freeze him (wont touch him until after event, dont want to use any items/chrons that could affect my ranking). (I do have about thirty 5* not FE, but thats another list)
Really the only crew I dont "have documentation for" are the weekly reward crew, which I always get 4/4 for. I update my list maybe once every two weeks or so. But I have found this an effective way to keep track of my partially fused (4*/5*) crew. (All the others I have are immortalized, and I have no fear of losing them.)
